How to Apply

For all students, the following documents must be provided before enrollment is complete:

1. Registration Packet and Registration Fee
2. Birth Certificate (Official Copy)
3. Baptismal Certificate
4. Immunization Records
5. Diocese of Fort Worth Health Report
6. Copy of last report card (Grades 1-8)
7. Copy of most recent standardized test scores (Grades 2-8)
8. Transcript request form, signed (Grades 2-8)

We encourage families to apply online as it simplifies submission and provides parents the ability to track the status of their child's application. To begin the online application process:

Upon account creation, you will receive an email containing a verification link. Once your email address has been verified, log in to your account and "Create A New Student Application" for your child. You will then have the flexibility to log in and out of your account and access your open application.
